To the right, you will requested information if you are receiving personal training services or nutrition services.

If you are using your private healthcare insurance to cover the expense of the visit, a copy of your ID and insurance card will be needed for your file.  All sensitive patient information is kept private and not release without your authorization.  Your primary care provider will receive information regarding our visit as necessary.  Your insurance company will be sent a superbill for re-imbursement of nutrition services.
